Community Coordinator (50%)



Position Duties and Responsibilities

Ø Promote home care services and home health aide employment opportunities at local businesses, faith and community based organizations, and local expos and fairs.

Ø Assist management in the development of linguistically and culturally appropriate outreach strategies to increase home care service utilization in the South Asian community.

Ø Work with administrative staff to develop bilingual outreach materials.

Ø Collaborate with government, independently owned, and faith and community based organizations to ensure seamless delivery of home care services.

Ø Provide written and verbal interpretation services as needed.

Ø Aide HR in hiring of new home health aides.

Ø Set-up and maintain client records and aides’ schedules.

Ø Communicate and maintain relationships with Area Agencies for Aging.

Ø Download and maintain service authorizations from counties and independent coordination agencies.

Ø Attend and contribute to agency meetings and represent the agency at community events.

Ø Assist in organization and recruitment of direct care worker certification classes. 

Marketplace Navigator Position (50%)


Duties and Responsibilities

Ø Distribute and publish language appropriate and culturally relevant information on health insurance and coverage to South Asian Americans in the community.

Ø Organize two (2), one-hour language appropriate and culturally relevant health insurance educational community workshops in the Philadelphia area.

Ø Recruit and enroll South Asian individuals in health insurance through CHIP or Medicaid (State Medical Assistance), or the Special Enrollment Period of the Health Insurance Marketplace, based on their eligibility.

Ø Provide translation services for program materials, flyers, and evaluation materials.

Ø Attend necessary trainings and workshops on health insurance, Marketplace, Open Enrollment, CHIP enrollment, and Medicaid enrollment arranged by PASSi.

Ø Attend outreach events and conduct educational workshops in the South Asian community.

Ø Maintain an accurate count of all individuals assisted with health insurance enrollment by retaining a copy of their signed consent forms.

Ø Help existing clients with information, eligibility determinations, or reporting change in circumstances for the clients.

Ø Follow-up with clients on the submission of documents, and during Open Enrollment.

Ø Represent clients in Marketplace appeal cases, including submitting appeals and for appeal hearings.

Ø Track total amount of informational sheets and flyers distributed during outreach.

Ø Collect all consent forms for each enrollment and submit to Navigator Program Manager.

Ø Report to Program Manager once a week by submitting weekly reports of health insurance enrollment and outreach events.

Ø Attend and contribute to monthly Navigator meetings.


Ø A minimum of an Associates or Bachelor’s degree required.

Ø A minimum of 12 months experience conducting community outreach and/or health education preferred.

Ø Demonstrated knowledge of the local South Asian community required.

Ø Ability to build trust quickly with community members and agency partners.

Ø Excellent customer service skills.

Ø Ability to work as part of a team or independently as duties require.

Ø Bilingual proficiency in English and at least one South Asian dialect is mandatory such as Hindi and/or Gujarati required.

Ø Strong communication skills, both verbal and written.

Ø Willingness to work some evenings and weekends.

Ø Strong proficiency with Microsoft Office Suite.
